Honeoye Falls-Lima Cougars Youth Football & Cheerleading

All participants will need to submit the following items with their registrations:

• Recent picture
• Physical form dated 8/1/2010 or later

All participants new to our program must also submit a copy of your birth certificate. If the parents' social security numbers are listed on the certiciate, please black them out on the copy.

These items can be uploaded electronically through our online registration system or mailed to our PO Box. We MUST have them on file on the first day or practice.
